Supportive Housing

Our McGrath St. Property has 14 beds in a congregate setting for low-income, chronically homeless individuals who need moderate levels of support. The program’s goal is to promote recovery at the highest level toward independent living.

Supportive Services Include:

  • ADL (Activities of Daily Living) task reminders and support
  • Medication Prompts
  • Case Management
  • Medication Management & Prescriber Services
  • Peer Support Services
  • Independent living skill groups

Housing Programs

Housing Bridge
Subsidy Program

A state-funded housing subsidy program through the Department of Health & Human Services. LRMHC manages the Bridge Program for the Belknap and lower Grafton counties, and provides housing services for individuals who meet the criteria of the program to obtain and maintain independent housing in the community.

Integrative Housing
Voucher Program

A state-funded housing voucher program through the Department of Health and Human Services. LRMHC manages the Integrative Program statewide, and provides housing specialist services for individuals whom have criminal histories that would otherwise disqualify their ability to obtain housing services through traditional housing programs.

Lakes Region Mental Health Center is proud to support Mental Health Month every May. This year, we are excited to announce that we will be hosting a Mental Health Art Auction. We are currently looking for local artists to help support this event.
