Care Transition Team Lead

Sara Bresslin is currently the Care Transition Team Lead. Sara came to LRMHC in 2020 as an intern, and became a therapist on the Neurocognitive and Older Adult Team after graduating and completing her internship. As the Care Transition Team Lead, Sara oversees the coordination of care for children and adults being discharged from inpatient psychiatric stays to services at LRMHC. She also supervises the Critical Time Intervention (CTI) team, an evidence-based program that supports adults being discharged from inpatient psychiatric stays to community resources. Sara also one of the Academy trainers for new clinical staff and interns, and is a Certified Nonviolent Crisis Intervention trainer.

Prior to joining LRMHC, Sara was the Education and Training Director for with the Life of an Athlete Program, an evidence-based youth alcohol prevention program with the New Hampshire Interscholastic Athletic Association (NHIAA). Before working with the NHIAA, Sara had experience working with college students as an Area Coordinator/Judicial Officer/Protective Custody Monitor at New England College (NEC), and has experience working in the finance sector.

Sara was led to the field of Mental Health by her passion to help others as well as her desire to make access to mental health treatment easier and more equitable. She saw the increased need for mental health services and supports throughout her varied work experience and decided it was her vocation. Her current role helps feed this drive as she primarily works with folks accessing treatment for the first time, and she continuously humbled to be a part of people’s journey to getting well.

Sara attended University of New England (UNE) for her undergraduate studies, and New England College for a Master’s in Business Administration. Later, she went on to earn a Master of the Arts in Clinical Mental Health Counseling from Southern New Hampshire University, where was named a Distinguished Scholar for having the highest GPA in her program! She was also a member of the National Society of Leadership and Success.

When Sara is not working, she can be found in community theatre productions throughout the state of New Hampshire, or avidly cooking and trying new foods.
